BEFORE YOUR FIRST VISIT

We would like to see your health records before your first visit, particularly if there is a complicated or chronic health problem. Change your PCP if your insurance plan requires one.

NEW PATIENTS

TRANSFER / NEW TO AREA PATIENTS Before your first sick or well visit at New England Pediatrics, request that your child's medical records (especially vaccine records) be sent directly to our office or bring them with you to your first visit. The transfer of records may take several days or weeks, so please request these records at least three weeks prior to your visit. Your past pediatrician will supply you with the necessary HIPPA Authorization for Release of Records. Prior health records are  required before we can complete camp or school forms.

Please supply your insurance card along with the following completed forms prior to your first visit: Two important caveats for new patients: we cannot see new patients during our Stamford walk-in hours as these appointments do not allow for thorough new patient assessment and we cannot accept a new patient whose family does not permit routine childhood vaccinations. Thank you for your cooperation.
