A doctor is available for URGENT CARE on weekend and holiday mornings by appointment. We also offer newborn visits during these times as needed. Please call at 8:15 AM to request an acute care or newborn appointment.

Such as medication refills, scheduling office appointments, or other non-urgent medical issues should be addressed when the office is open.
Refills will not be called in after hours. If you have questions regarding dosing for over-the-counter medications, please refer first to the packaging, or refer to the online calculator below.

SCHOOL/CAMP/SPORTS
CAMP, SCHOOL,& ALLERGY PROTOCOL FORMS
To efficiently manage the enormous volume of school, camp and allergy protocol forms, we ask that parents complete the following prior to submitting forms:
- Patient information/demographics
- Allergies (include Allergist Protocol if applicable)
- Chronic medical conditions
- Past surgery or hospitalization
- Daily medications (name and dosage)
- Restrictions in diet or physical activity
Allergy protocol, school and camp form fees must be paid upon submission: $15.00 per form per patient, or $30.00 for two or more forms when submitted simultaneously.
