Form Fee Update – Effective March 1st, 2025

We wish to inform you that our form fee will increase to $15.00, effective March 1st, 2025.

This is the first fee adjustment in over 20 years, and it reflects the substantial staff time required to process each form.

Please note that payment is required at the time of submission or when requesting forms by phone. Forms will not be processed until payment has been received.

Processing Times:

• Expect a processing time of 7-10 business days. Please note that turnaround times may be quicker between October and April.
• All forms will be automatically made available on the patient portal. If you'd prefer to receive them by mail, please supply a stamped, self-addressed envelope. Alternatively, you can pick up the completed forms directly from our office. Please note that we are unable to fax forms to schools or camps.

All non-urgent questions such as medication refills, scheduling office appointments, or other non-urgent medical issues should be addressed when the office is open.                                   

Refills will not be called in after hours. If you have questions regarding dosing for over-the-counter medications, please refer first to the packaging, or refer to the online calculator below.
